• at the time of submitting your application, a period of at least six (6) months has elapsed since your last physical fitness crisis.
• you are or are subject to a postponement of health, have been granted an annual or two-year deferral rather than a six-month deferral.

You will need your personal Taxisnet credentials.

You may optionally attach a medical certificate showing an improvement or deterioration in your state of health or a solemn declaration showing the above.
