If you have fallen sick in a member state of the European Union or in a third country, you can apply to have your disease registered in the register of COVID-19 patients.

You will need to:

• enter your SIN /T-SIN /PAAYPA
• enter your ID card or passport
• attach certificates or attestations with your disease information in digitally verifiable form and that they have been issued in an EU member state (EU Digital COVID Certificate - EUDCC) or in a third country, for the COVID-19 certificates of which an executive act of the European Commission has been issued

Once you have registered, provided that the criteria set by the National Vaccination Committee are met and verified by the application, you can either:

• get a National Certificate of coronavirus disease

• or schedule your booster vaccination dose
