• upload the necessary supporting documents in electronic form
• enter the details of your bank account (IBAN), in which you are sole beneficiary or co-beneficiary
• enter your mobile phone number

After logging in to the service, select "Personal benefit applications" and then "Submit" and "Travel expenses of kidney disease patients".
