You may request that you be referred to an Exemption Committee for a review of your physical fitness if you have been found unfit for Ι/5 by the Armed Forces Health Commissions and a period of at least six (6) months has elapsed since your crisis as unfit.

When you apply
The application shall be submitted at any time until you reach the age of forty-five (45) and if a period of at least six (6) months has elapsed since your assessment as inappropriate.

You will need your personal Taxisnet credentials.

You may optionally attach a medical certificate showing a cure or improvement of your state of health or a solemn declaration showing the above.
